Food Establishment Plan Review

New food establishments intending to operate in Maplewood must first apply for a plan review. This requirement also applies to existing food establishments undergoing remodeling, layout, equipment, menu, or ownership changes. For new establishments or establishments changing ownership, a Food Establishment license must be applied for and issued after the plan review has been completed, and before operation can begin.

Applying for a Plan Review - APPLY NOW

The Food Establishment Plan Review application is available through our Citizen Access Portal at www.MaplewoodMN.gov/ACA. Please note that applications are reviewed on a first-come, first-served basis and only after all application requirements are met. Once your application has been deemed complete, please allow up to 30 days for processing.

The items listed below are required. Documents can be uploaded to the online application or emailed to the Environmental Health Officer at Molly.Wellens@MaplewoodMN.gov

  1. Application Fee - assessed by staff after review of application. You will be notified when the invoiced fee is available to be paid.
  2. Equipment Schedule for each piece of equipment
  3. Manufacturer Specification sheet for each piece of NEW equipment
  4. Finish Material Schedule
  5. Elevations and drawings for all custom-fabricated equipment
  6. Cabinetry and countertop finish information
  7. Intended menu
  8. HACCP or TPHC plan (if serving raw animal products, using specialized processes, or using time as a public health control)
  9. Easily readable layout to scale, including: 
    • Location of equipment 
    • Hand sinks, 3-compartment sink, and prep sink
    • Bathrooms
    • Storage areas
    • Wait stations
    • Bars
    • Janitor stations
